    Skills

    Knowledge:
    Knowledge of professional social work principles, methodology, and ethics and of human psychosocial
    development within the family, community, and culture.
    Knowledge of the use of therapeutic relationship to foster patient involvement. Familiarity with brief
    therapy theories/techniques and therapeutic process. Understanding of techniques for facilitating client
    motivation to change behavior.
    Knowledge of health and social issues impacting diverse clients and their well-being.

    Minimum Qualifications

    Education and/or Experience
    Master’s degree in social work from accredited school of social work. Additional course
    work in area of specialty (e.g., psychiatric, pediatric, geriatric) preferred.
    Minimum two years of experience in social work and broad background with diverse
    patient populations, preferably in medical setting. Additional one year of experience in
    area of specialty preferred.

    Specific Areas of Responsibility

    Conducts comprehensive, culturally sensitive psychosocial assessments, develops care plans, and
    provides counseling and crisis intervention services to individuals and families.
    Coordinates services with appropriate clinic and community resources.
    Provides information and education about health/mental health issues to individual patients and to
    groups.
    Documents assessment, intervention, and treatment data in medical record.
    Serves as liaison to community health care network.
    Works closely with Care Management team to coordinate services for empanelled care
    management patients.
    Other duties as assigned

    General Physical Requirements

    While performing the duties of this job, the employee is regularly required to stand; walk;
    use hand to finger, handle, or feel; reach with hands and arms and talk or hear. The
    employee is occasionally required to site and stoop, kneel or crouch the employee must
    regularly lift and /or move up to 10 pounds, frequently lift and/or move up to 25 pounds
    and occasionally lift and/or move up to 100 pounds.
