Job Description

Pacific Medical Group, of Praxis Health, is seeking a motivated team-player to work as a Behavioral Health Consultant Care Navigator (BHC-CN) on our primary care behavioral health team. This is a full time position with an excellent benefits package. We’re seeking an individual who works well with people and has a strong work ethic.

 

This position can be located at any of our Pacific Medical Group clinics in the Portland Metro area (Oregon City, Beaverton, Tigard, or Canby)!!

 

The Behavioral Health Consultant Care Navigator (shortened to BHN) is healthcare extender that guides patients through and around barriers in the complex healthcare system. As a health professional BHN will provide support to overcome obstacles faced in accessing or receiving services needed to maintain well-being & independence. BHN will provide practical support services to patients with complex medical and mental health conditions, frail patients, and/or patients facing homelessness or other stressors. Main function of this position is to facilitate patient’s access to services that will increase better health outcomes.

 

The role of the BHN is to assist the team with on-going care for patients who are most at risk for poor health outcomes. This will involve development and maintenance of patient registries and initiation of on-going contact with identified patients until gains are made. The BHC Care Navigator will coordinate care with external prescribers of psychotropic medications or other specialty mental health providers.

The responsibilities of the BHC Care Navigator include the following:

  1. Is accessible by phone or in person during all clinic hours (8am-5pm M-F).
  2. Is available to primary care patients and health professionals on a same-day basis.
  3. Responds to referrals from the PCP or others.
  4. Establishes close relationships with and serves as primary point of contact for clients.
  5. Coordinates with other BH navigators and BHCs in the system (Praxis).
  6. Administers standard screening and outcome measures instruments.
  7. Supports patient practice of skills learned in consults with the BHC.
  8. Assists the BHC in delivery of group visits, if available.
  9. Provides support to the PCBH pathway programs in constructing and maintaining registries, preparing evaluation summaries, completing independent outreach to patients.
  10. Provides patient education and community resource materials to patients.
  11. Manages and coordinates referrals to specialty behavioral health services including MH and SA services.
  12. Assists patients in meeting basic needs such as food, shelter, clothing, benefits.
  13. Identify resources for clients to overcome barriers to care, such as transportation, housing, and childcare arrangements.
  14. Maintains up-to-date contact information about commonly-used community resources.
  15. Manages and coordinates referrals to community resources.
  16. Supports patient in obtaining durable medical equipment.
  17. Supports patient in obtaining legal services and benefits advocacy services as requested by the PCP or other staff.
  18. Completes selected sections of commonly used forms.
  19. Represents PCBH Program staff in meetings with local community groups and governmental and social agencies to provide information on the activities and goals of the PCBH program.
  20. Maintain strict confidentiality in accordance with agency policies
  21. May meet with clients after primary care provider appointments to review and update care plan.
  22. Meet with Care Team (including, but not limited to, primary care provider, MA) to discuss client care issues and needs and facilitate client health care
  23. Maintain documentation of all client encounters and complete reporting requirements according to organization standards
  24. Track client information, schedules, files, and forms in a confidential manner
  25. Track client attendance at medical appointments and patient navigation sessions and initiate outreach and missed appointment procedures, as necessary
  26. Attend and represent the organization at professional conferences, in-service trainings, and meetings at the request of or with the approval of supervisor
  27. Reliable attendance and presence at work.

Minimum Qualifications

Preferred Qualifications, Personal Characteristics, and Skills:

  1. Commitment to the mission of care coordination
  2. Passionate, trustworthy, and empathetic when working with clients
  3. Ability to build relationships with different types of people, including clients, organization members, and health care providers
  4. Good communication and interpersonal skills and ability to speak concisely to clients and team members
  5. Organized with confidential client material and appointment tracking
  6. Flexible and adaptable in response to changing client and health care providers’ needs
  7. Interest in working with patients with chronic health problems and behavioral health needs

Education and experience:

  1. Minimum of 2 years college education; BA/BS preferred
  2. Experience in medical setting preferred
  3. Strong understanding of cultural competency with patient population
  4. Computer literacy and familiarity with MS Office software
  5. Exposure and comfort working in medical setting, with medical and psychiatric issues, and issues of death and dying

General Physical Requirements

  1. Manual dexterity for office machine operation including computer and calculator; stooping, bending to handle files and supplies, mobility to complete errands for deliveries, or sitting for extended periods of time.
  2. The employee must regularly lift and /or move up to 10 pounds, frequently lift and/or move up to 25 pounds and occasionally lift and/or move up to 50 pounds.
  3. Must be able to sit, stand, kneel, crawl, bend and twist as needed.  Requires walking on non-level surfaces, inside and outside the office, throughout the day.
  4. Stress can be triggered by multiple staff demands and deadlines.
  5. Work is performed in an office environment. Involves frequent contact with staff and patients.
