Are you a skilled and compassionate Behavioral Health Consultant (BHC) looking to make a meaningful impact on patients' lives? Praxis Health is seeking a dedicated professional to join our integrated healthcare team in Bend, Oregon.

This full-time position offers a rewarding opportunity to work alongside primary care providers in delivering holistic care that addresses both physical and mental health needs within the Primary Care Behavioral Health (PCBH) model.

As a BHC at Praxis Health, you will play a crucial role in enhancing our patients' well-being, working in a supportive, collaborative environment that values innovation and continuous improvement in patient care.

 

Key Responsibilities:

  • Conduct evidence-based behavioral health assessments and provide personalized interventions in a primary care setting (in-person and via telehealth)
  • Serve as a trusted consultant to primary care providers, offering guidance on behavioral health strategies within the PCBH model
  • Collaborate with primary care providers to design and implement holistic treatment plans for patients
  • Provide targeted behavioral health support for issues like depression, anxiety, stress management, lifestyle changes, and self-care
  • Deliver both short- and long-term evidence-based therapies, including the chance to develop group therapy and psychoeducational programs
  • Engage in interdisciplinary team meetings to coordinate comprehensive patient care
  • Maintain accurate patient records and stay up to date with trends and best practices in behavioral health

Minimum Qualifications

  • Educational Qualifications: Master’s degree in Social Work, Psychology, Counseling, or a closely related field.
  • Licensure Requirements: Current licensure as a Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Marriage & Family Therapist (LMFT).
  • Experience working in an integrated care setting preferred but not required.
  • Strong clinical assessment and intervention skills with a focus on brief, solution-focused therapy.
  • Ability to work collaboratively within a multidisciplinary team and communicate effectively with primary care providers.
  • Comfortable with using telehealth technology for remote patient care.
  • Flexible and adaptable to changing work environments and patient populations.
  • Commitment to providing culturally competent and trauma-informed care.
  • Will attend work regularly and on time, as scheduled.

  • Sitting: Prolonged periods of sitting for patient consultations and administrative tasks.
  • Manual Dexterity: Frequent use of a computer, keyboard, and other office equipment for documentation.
  • Communication: Strong verbal and auditory abilities to effectively interact with patients and colleagues.
  • Visual Acuity: Ability to read and interpret patient records and electronic documentation.
