Job Description

Oak Street Medical Primary Care is looking for a friendly and highly skilled Triage Registered Nurse to join our team.

 

Triage Registered Nurse will participate in the development and support of a cross-regional patient phone triage system within Praxis Health Southern Willamette Valley. The triage nurse will be asked to oversee, develop and refine the triage system. The RN will be responsible for phone triage and other related duties as demanded by the clinics. The RN will collaborate with the leadership and providers to expand the department scope of practice, ensure quality of operations, and provider expert insight into system needs for optional functionality and compliance with local, state and federal regulation and standards.

Essential Job Functions:

  • Monitors and responds to triage calls received via task, on the nurse phone line and through patient portal.
  • Using established protocols:
  • Talks directly to patients on the telephone and then directs them to the appropriate place for care of offers home care advice.
  • Determines urgency of seeing the patient based on protocol questions and incorporating data gathered form the patient’s chart.
  • Sends patients with high-risk chief complaints such as chest pain, abdominal pain, or severe headaches to ER immediately or arranges for ambulance.
  • Coordinates arranging appointments for patients who do not need to go the ER but need to see a clinician.
  • Consults with the clinician as needed to help determine best disposition for patient if unclear based on protocols.
  • Helps develop messaging for common information requests from patients, e.g., “do I need a flu shot every year? Etc.
  • Serves as a clinical resource and professional role model for the clinical team.
  • Prioritizes work tasks and completes in a timely manner.
  • Assists in developing policies and procedures related to position.
  • Communicates appropriately and tactfully with leadership, staff, facilities, patients, and community.
  • Reduces conflict and increases patient satisfaction whenever possible through timely responses.
  • Participates in staff, planning, in-service and other meetings as needed.
  • Performs other duties as assigned.
 

Minimum Qualifications

  • RN License
  • Minimum two years in direct patient care, preferably in outpatient care setting.
  • Excellent verbal and written communication skills, including strong organizational, detail and interpersonal skills.
  • Competent computer skills and knowledge of HER systems.
  • Excellent problem-solving skills.
  • Innovative, forward-thinking mindset.
  • Demands high work from self and others.
  • Demonstrated ability to work effectively with diverse races, ethnicities, ages, and sexual orientation in a multicultural environment.

General Physical Requirements

  • While performing the duties of this Job, the employee is regularly required to stand; walk; use hands to finger, handle, or feel; reach with hands and arms and talk or hear.
  • The employee is occasionally required to sit and stoop, kneel, crouch.
  • The employee must regularly lift and /or move up to 10 pounds, frequently lift and/or move up to 25 pounds and occasionally lift and/or move up to 50 pounds.
  • Work Environment: Combination of medical office and exam/procedure room settings.

This position performs tasks that involve exposure to blood, body fluids and/or tissues. All employees performing these duties are offered the opportunity to receive the Hepatitis-B vaccination series.
